Appointment Requests
Please complete all appointment requests by downloading our appointment request app for on the go convenience (scan the QR code below) or at the bottom of this page . In the notes section on the form, please provide patient's, name, MRN, DOB, LMP or EDD. We use a report portal for all reports. Please let us know if you do not have a portal username and password yet. All of your patients are able to self schedule their appointments at www.preciouspeeks.org.
